To resolve this issue, please follow this steps:
1) Delete the row with the old server from the dbo.MetalogixLicenseInfo table
2) Restart the Metalogix Diagnostic Manager Collection Service in Windows Services
3) Open the Diagnostic Manager console and re-activate the license. This will add a row to the table with the correct server information.
This will put in the current server and license information into that table and will stop the prompt for Activation.
