SQL database creation fails with error code -2147467259 because the install of SQL Express, as part of the Recovery Manager for AD install, has failed.
"Error - 2147467259: failed to create SQL database: RecoveryManager-Reporting-XXXXXXX, error detail: unknown error."
The installation of SQL Express 2008 may have failed with the following error:"SQL Server installation failed with Exit code (Decimal): -2068643839"Exit code (Decimal): -2068643839Exit facility code: 1203 -
Resolução
The issue is related to certain rights and permissions granted to the local administrators group. If these have been altered to tighten security then the installation of SQL Express may fail.

This issue was observed in older end of life OS versions like Windows 2008 R2 when attempting to install SQL Express 2008 and SQL Express 2012.
