Performance Analysis for SQL Server (PASS) is reporting that the "Monitored Instance is Down" when in fact, it is still up. We are running a version of SQL Server which is not on the supported list per the 6.5.3 release notes. The version changed through an automatic update.
Upgrade from 6.0 to 6.5 did not complete successfully. The PASS Collector does have the capability to connect to DB shared memory though the SQL Server version may not be certified in the Release Notes but that observation applies to PASS 6.5 version onwards.
In a rare case it can happen that if the upgrade from 6.0 to 6.5 does not complete successfully, the links to the new executables might not be created. For example, if you look in the <install_dir>\agents\SC_MSSQL\6.0\bin directory you might see something like the following:

As you can see, the filesize of quest_sc_mw.exe matches the quest_sc_mw.exe@25 version (6.0.25), not the quest_sc_mw.exe@309 version( 6.5.309).
The solution is to copy the @309 binaries to the .exe. In the above example, you would copy quest_sc_mw.exe@309 to quest_sc_mw.exe so that it looks like this:

Repeat the above process for each executable that did not get poperly upgraded (quest_sc_collector.exe, quest_sc_mw.exe, quest_sc_rep.exe).
