After a disk failure, system goes in maintenance mode with the following reason:
"Filesystem server is crashing repeatedly in Maintenance Mode."
Dell PERC 9 controllers (H330, H730, H730P, and H830) introduced a feature called Rapid Rebuild that speeds up the time to rebuild failed drives in certain conditions.

PERC9 –Under certain conditions, there is a possible data integrity issue with the Rapid Rebuild.
In order to make the system operational, it's necessary to fix the filesystem:
Run a filesystem check with the following command
maintenance --filesystem --scan_now
to check the scan status use this command
maintenance --filesystem --scan_status
Once the scan is completed, type the following command to check the scan report
maintenance --filesystem --scan_report
If errors are reported, run the filesystem repair with the following command
maintenance --filesystem --repair_now
If the scan fails, please contact Quest Support.
How to prevent the issue:
Latest DR firmware contains the latest PERC firmware which have the rapid rebuild function disabled.
If DR OS version is 3.2 or earlier, please upgrade to the latest available firmware.
