Cause
Failure to complete FMS install which led to security issues in the repository database.
From the install log file:
Install Action: FoglightDatabasePanel
Configuring the foglight database...
Connecting as the database administrator...
Database Install Failed
Failed to Connect: Failed to connect to the database as administrator user:ssfgsad1Status: FATAL ERROR
Numerous messages like the following were in the FMS log:
The server principal "foglight" is not able to access the database

Resolution
Foglight user did not have permission to create tables on a portion of the database. This was corrected by Quest Support and the issue was resolved.
