This issue occurs after installing Toad for Oracle 12.8 on a system where a previous installation of Toad for Oracle 12.1 is present .
In this scenario, the application data directory of the 12.1 version was previously changed from the default path (C:\Users\<USERNAME>\AppData\Roaming...) to the local default (C:\Users\<USERNAME>\AppData\Local...).
The user opens Toad for Oracle 12.8 and navigates to View | Toad Options | General and clicks on "Use local default". Clicking on "Apply" generates the following error message:
"Toad for Oracle has encountered a problem. We are sorry for the inconvenience.
Access violation at address 000000000854B964 in module 'Toad.exe'. Read of address 000000000000002C.

This error also occurs when attempting to set the application data directory to a different path than the local default one.
The following steps can be used to resolve the issue (before taking these steps, please make a backup of the Toad for Oracle 12.8 roaming folder.):
1. Rename the Toad 12.1 user roaming folder:
C:\Users\<USERNAME>\AppData\Roaming\Quest Software\Toad for Oracle\RENAMED_12.1
2. Rename the Toad 12.1 user local folder:
C:\Users\<USERNAME>\AppData\Local\Quest Software\Toad for Oracle\RENAMED_12.1
3. Delete the user roaming folder:
C:\Users\<USERNAME>\AppData\Roaming\Dell\Toad for Oracle\12.8
4. Run Toad 12.8. This creates a new 12.8 roaming folder with no saved connections and all settings set to default.
5. Create a connection to a database.
6. Restart Toad 12.8.
7. Navigate to View | Toad Options | General and click on "Use local default".
8. Click on "Apply", a confirmation message will pop up.
The application data directory will now be changed successfully.
Please refer to SOL42583 for more information on migrating user settings across from previous Toad for Oracle versions.
