Master Detail Browser. If there's an error or mistake in the Filter, child tables are removed or deleted and work is lost.

To replicate the issue, open ‘Master Detail Browser’ and add a main table and 1 or 2 child tables. Issue happens when there is an error in the Filter i.e. typed 1=a instead of 1=1 (and two errors will appear).  When user goes back to the Filter to correct the problem, the Child Tables will become blank. Un-saved work will be lost, and the Child tables will need to be added again.

WORKAROUND:

Perform a 'Save' more often and try not to have errors in your filters.

Issue fixed in version 12.9. The latest version of Toad for Oracle can be downloaded here.
