Description
When SQL Tuner is launched the following error is returned:
ORA-12154: TNS:could not resolve service name -
Cause
In order to use SQL Tuning, the SQL Tuning option must be on the license key entered in Toad.

Resolution
Check to make sure the license key being used has the SQL Tuning option on it. The license key options can be verified by selecting Help | About in the Toad menu bar.
