Cause
Conflict with other version of QSA

Resolution
1. Connect to database, run the following:
delete quest_com_products
where product_id = '61';
commit;
2. Make sure no qexecd process is running
3. Remove QSA installation directory
4. Clean up the staging area
5. Reftp the tar.z file
6. Untar it
7. Run Install.sh
