Cleaning media was loaded into an LTO-4 drive, then the tape library tries to perform some cleaning and then hangs in the drive i.e. cleaning media doesn't return back to the allocated slot.
There were no errors logged within the Binary log, but there were errors logged within some Trace files.

The problem was that an LTO-1 and LTO-2 cleaning tapes were used within an LTO-4 drive and is not supported i.e. you can only read/write one generation back and can only read two generations back.
Since cleaning information has to be written to the cleaning media chip within the cartridge, you can only use an LTO-3 or LTO-4 cleaning media in an LTO-4 drive.
Note: Make sure that your tape library is also configured correctly for cleaning media as per the KBA 78529 "How to configure Supported Libraries to use cleaning media".
