For SQL Server, the table/column comments are missing in the Forward Engineered syntax.
Resolution

Description: When executing Forward Engineer schema how do I generate MS SQL Server DDL, Extended property "Comments" for both Tables and Columns.

Solution:

After creating the model, right click on the new table and go to Table Properties

Select the schema owner name.

Under Actions | Forward Engineer | Schema, go to other options and check Comments.

Then when you preview the Schema and you will notice the comments are now showing.
