Resolution
Upgrade Toad for Oracle to the latest version.
The latest version of Toad for Oracle may be downloaded at:
http://support.quest.com/support_download/Downloads.asp
If on current version, uninstall and reinstall Toad for Oracle using the Quest Installer. See Solution SOL36982 for details.
