I assume that "true" means that the notification is enabled, "Warning" is the lowest Severity level, and that Alert Log Message @Severity@ on @InstanceName@ is the Email message subject. test@domain.com is our Email address. I understand that the @InstanceName@ is the database instance name and @Severity@ is the alarm's severity level.
