ODM has the same device duplicated in a different enviorement. This could be as a result of uploading the file to an incorrect Enviorement when using ODM Express or in ODM Directory Sync due to a workflow that is incorrectly set up.

Search in all the Environments the device that is being reported as duplicated. Once you locate the device to remove it you will have to delete it in the Active Directory or move it to an OU not in scope of the environment and then run a Read with Reconcile selected.
