The meeting existed in Notes prior to migration, the meeting is then migrated to Exchange. The organizer on Notes then cancels the meeting but on the Exchange side, the appointment is not being removed from the Exchange user's calendar.

A user that has been migrated to Exchange, updates a meeting created from when they were in Notes, all attendees of the meeting on Notes receives a new meeting request from the user with the new updates instead of updating the already existing meeting entry.

This issue has been acknolwedged as a bug and will be addressed in future versions of Notes Migrator for Exchange.
