This issue was fixed on the latest release Change Auditor 7.2 Full Maintenance Release (build 7.2.0.24021)

Workaround in case upgrade is not possible straight away:
- On the Coordinator server, launch notepad as an administrator
- In Notepad, open the AzManXmlStore.xml file, which is typically located in the Coordinator Service folder: "C:\Program Files\Quest\ChangeAuditor\Service\"
- Scroll down until you find the section labeled: <AzTask Guid="e8caea98-a8aa-4eed-9014-3e1214f66a65" Name="Operator Task" Description="Grant Operator access only." BizRuleImportedPath="">
- Add the following two lines to the file after the above tag, and before the closing "</AzTask>" tag:

<OperationLink>dbd64891-cfa2-442d-a6be-65866ca901c3</OperationLink> - Save and close the file
- Restart the coordinator service
- Repeat this process on each Coordinator that exists.
