Please try the steps below to resolve the issue:
1. Exit Toad
2. Go to the Toad directory where the Toad file are store again c:\user\user_name\app data\quest software\Toad for Oracle\User Files
3. Delete the following two files:
EditorDesktopLayout_PLSQL.lay
EditorDesktopLayout_SQL.lay
4. Launch Toad, it will recreate these files for you.
