SQL Profiles are suggested by Oracle after tuning a statement.
Within Toad, open Database | Optimize | Oracle Tuning Advisor (OEM). In the Tuning tab, enter in the long running SQL. It will run and produce a report below, in the "Report" tab. In there, Oracle may recommend a SQL Profile. If it does, it will note how to accept this profile. Run the statement to accept the profile as recommended by the Oracle report.
Once that is run and accepted, the SQL Profile will be listed in the SQL Profile tab with in Toad. From there it can be managed within Toad.
If a profile is not recommended by Oracle, there is no way in Toad to manually create one.
