ApexSQL Subscription based products - Failed to Login Internet Connection is required to continue (4362314)

After installing ApexSQL Subscription based product(s), the following error is displayed when entering credentials or the Microsoft login window is blank:

Enable JavaScript

1. Click Tools > Internet Options
2. Click the Security tab > Custom Level
3. In the Scripting section, click Enable for Active Scripting
4. In the dialog box that displays, click Yes

Enable cookies

1. Click Tools > Internet Options
2. Click the Privacy tab > Advanced
3. In the newly opened dialog, choose to Accept both “First-party Cookies” and “Third-party Cookies”
   1. If this is already set to “Accept”, change it to either “Block” or “Prompt”, save, then change it back to “Accept”
   2. If the above doesn’t work, try disabling the “Always allow session cookies” as well
