On the SQL Server end:
1. Right click on the SQL Server connection of interest and choose "Properties"
2. Under host, choose "Do not monitor" from the drop down menu
3. Click OK all the way out
4. Right click on the SQL Server connection again and choose "Disable monitoring"
On the Windows end:
1. Right click on Windows connection that hosts the SQL Server of interest
2. Choose "Disable monitoring"
Delete from the Connection Manager:
1. Right click on any connection on choose "Connection Manager"
2. Go to the SQL Server section and press delete (or right click delete) for the SQL Server of interest
3. If not keeping the Windows host, go to the Windows section and do the same
