Description
Session failed:ORA-14400: inserted partition key does not map partition. Partition table gets ORA-14400 because there are new partitions added.

Cause
This occurs when Reorg fails due to new partitions added during reorg which were not present when script started.

Resolution
1) Cleanup the quest temp table in the temp tablespace
2) Get rid of the trigger
3) Run reorg when data definition language (DDL) is not going on.
