When discovering a RAC database and after successfully finishing connection validation information of a node is missing and a node information is duplicated twice.
Agent status will show all nodes and RAC information appropriately.

Node for which dashboard information is missing might be down.
Run "select * from gv$instance;" logging in a sysdba or system and make sure all nodes are showing up and availbale.
