Error: SQL Server does not exist/Access Denied (4284201)

Error: 

[DBNETTLIB] ConnectionOpen (Connect()).]SQL Server does not exist or access denied. 

Symptoms:

The main cause of this error is due to no access/role permission to the database. 

Resolution:

Please test the database connection by:

• On the log on box, please select “Edit..” (please make sure that your database location is pointing to the correct database name)
