When running an Explain Plan with the Editor, Toad for Oracle returns Access Violation errors.

Resolution
Determine if the Access Violations are reproducible.
Close all instances of Toad for Oracle and execute the Explain Plan again.If the Access violation is returned again, open a support SR via online Support Portal.
Include the steps taken, a screenshot of error and the support Bundle.
