When attempting to use the "Add Backup" to add a backup file either locally or remotely (via UNC path), the following prompt may be returned:

In "C:\Path\0000001C.bak" backup file:
LiteSpeed 2005 could not open the Backup file:
(\\ComputerName\C$\Path\0000001C.bak). The previous system message is the reason for the failure.

This is due to the SQL Server Service Account. If the SQL Server Service Account (on the Site Administrator for SharePoint/Recovery Manager for SharePoint) is set to "Local System", the "Local System" account cannot make network calls which are needed for this action to complete.
The end result is the "Add Backup" action fails with an Access Denied.
For the SQL Server Service Account that houses the QSP_Repository database, please specify a domain account that has access to the folders (via UNC path) that contain the backups via the Services applet. It is best to close the Recovery Manager for SharePoint console application and restart the SQL Server Service before proceeding.
