"Had media error" and "Failed to get restore stream" errors when migrating a phase 1 backup off... (4046464)

Performing a Phase 1 backup to a NEXSAN RAID 0 SVTL device that is attached to the NetVault Server via FC and then a Phase 2 Duplication-Migrate option to a Spectra Logic T120 tape library that is attached to a separate FC card also on the NetVault Server.
Confirmed the following error messages:
