User has a save SQL file on their computer. In Toad, they go to File | Open. The file opens into the Editor, but the Editor's Tab for this file does not contain the name of the file, it stays "<No name>" Trying to save the file does not work, you must do a Save as and enter the file name again.
The toad.ini file has been corrupted. Do the following to create a new one. Warning, this will cause you to lose your saved settings and passwords.
1. Shut down Toad for Oracle.
2. Browse to (default) C:\Program Files\Quest Software\Toad for Oracle\User Files\Toad.ini.
3. Rename Toad.ini to Toad_OLD.ini
4. Launch Toad for Oracle and it'll create a new Toad.ini file.
5. Connect to the database and re-test the issue.
