Connection Login Window is missing: "Drag a column header here to group by that column section"

Corrupt file
1. In the Connection Login Window icon bar, export the connection strings via the Export icon.
2. Backup the Toad for Oracle Application data directory version folder
3. From Toad for Oracle main menu: View | Toad Options| General >>> Application data directory section Click on “Open Folder” button.
4. Close Toad for Oracle
5. Go back to the Toad for Oracle Application data directory version folder
6. Rename the version folder to back up (I.E. Toad for Oracle 11.6 to 11.6 backup).
7. Launch Toad for Oracle.
8. A new default Toad for Oracle Application data directory version folder will be created.
9. Confirm that Connection window’s user’s group by panel is available again.
10. In the Connection Login Window icon bar, import the connection strings via the Import icon.
