Automation Designer | Action Recall does not recall all previous actions. It only appears to recall 49 actions.
Toad limits the number of saved Recall Actions to 50. Using View | Toad Options menu option and navigating to "General" section, value 50 in "Save <50> Toad Actions per action type" setting is the limit on this setting.
