CAUSE 1
For MySQL servers versions below 5.6.5 the the slow query log must be configured for the MySQL agent
CAUSE 2
The performance_schema is not configured on the MySQL server instance or collection not enable on the MySQL Foglight agent
CAUSE 3 and 4
Due to amount of information on the monitored instances, related topology objects in Foglight are reaching their limits
CAUSE 5
Permissions issues
CAUSE 6
Advanced Filter is on. By design the agent will filter the Top SQL coming from performance_schema, information_schema, mysql, sys database, etc.

RESOLUTION 2
Confirm that the performance_schema is enabled, the following command can be used:
show variables like 'performance_schema';
RESOLUTION 3
Increase the topology limits for the object "MySQLAgent_Statement_Digest". Additional information on how to increase the limit is available in the following article:
How to increase Foglight topology instance limits? (76022)
Note that increasing the limits would cause higher memory usage on the Foglight Management Server due to increase in the amount of topology objects being created.
RESOLUTION 4
Apply changes on the "performance_schema_digests_size" MySQL variable to limit the amount of row in the events_statements_summary_by_digest table or disable the agent collections for instances that do not need them. This would reduce the amount of topology objects being created in Foglight.

RESOLUTION 6
Click the red X (Advanced Filter On, bottom right" to collect topology data collected from MySQL
NOTES
After make changes, execute this statement to get new fresh data.
truncate table performance_schema.events_statements_summary_by_digest;
Then run the following to clear Mysql statement digest topology in Foglight.
