WORKAROUND #1:
Launch SQL Optimizer as a standalone product from the installation folder.
WORKAROUND #2:
This issue only occurs in the Toad for SQL Server 64-bit edition. It does not occur in the 32-bit edition. If the 64-bit version is un-installed, the 32-bit version can be installed and used.
