The "File" menu item appears or shows up six or 7 or several times in the main menu bar and/or icons missing from toolbar.

Resolution
Reset Toad to factory default settings by renaming the Data directory:
1. Open Toad for Sql Server.
2. Select the Help | About menu on the main menu.
3. In the About box, click on the Application Data Directory link, which brings up an Explorer window to that directory.
4. Close the About box by clicking the OK button.
5. Close Toad for Sql Server.
6. Note the directory name that is displayed in the Explorer window. Go up one directory in the Explorer and then rename.
7. Restart Toad for SQL Server and re test the issuePlease Note that your existing User settings will be lost in this process.
