Issue
We are attempting to migrate our users, and we receive the following error

We don't understand why the autodiscover credentials are blank. The Settings - Additional tab are configured correctly, and the Worker Connectivity Tester passes.
What is causing these migrations to fail?
Solution
The Migration profile you are using is malformed or corrupt. Go to Configuration - Migration Profiles view, and edit the migration profile(s) for these users.
Save the migration document
Go to the 'Set Migration Profile' view
Select the users, and reset the migration profile
Run a migration
