Three Azure tabs are displayed in the Alarm Templates after upgrading to Foglight 6.0. Two tabs are labelled Azure and the other is labelled Azure SQL.
The following rules from older SQL Azure or Azure SQL cartridges (including 5.5.8) which were supposed to be automatically deleted in cartridge versions above 5.9.5.10 remained in the FMS.

SQL_AZURE - Bandwidth Usage Baseline Deviation
SQL_AZURE - Database Collection Auto Stop
SQL_AZURE - Attach Alarm User Defined Data - General Rule
SQL_AZURE - Cost Changed
SQL_AZURE - Cost Was Not Set
SQL_AZURE - Database Property Changed
SQL_AZURE - Total Connections Baseline Deviation
These rules caused the presence of one of the two "Azure" tabs.
The second Azure tab was present due to the activation of an AzureAgent cartridge in the system. The tab however included the SQL_AZURE rules listed above because of a order of preference where the SQL Azure rules took priority over the AzureAgent rules, causing them to be hidden.
The "Azure SQL" tab contain the rules associated with the Azure SQL cartridge at a 6.0 and higher level.
Disable (and delete) the following deprecated rules from the Foglight Management Server. The topologies associated with these rules have been removed from the Azure SQL cartridge as of 5.9.5.10.

This should remove any tab labelled "Azure" that contains SQL_Azure rules.
Depending on the presence of an AzureAgent cartridge in the FMS, there may be an Azure SQL and Azure cartridge displayed after these rules have been disabled/deleted.
