Error after Azure SQL Cartridge upgrade from 5.5.x to 5.9.7.x -
Description
New Azure SQL DB cartridge 5.9.7.x installation fails due to licensing. Old SQL Azure cartridge is version 5.5.8. Attempts to monitor an Azure SQL DB generates this error message:
Error creating view "system:dbwc_azure_installer_simplemode.installerForm", please check application logs

Cause
Foglight SQL Azure Cartridge 5.5.8 is an unsupported retired product.
Foglight Cartridge for Azure SQL DB 5.9.7.x is a supported product, but requires additional licensing.
These two cartridges are not the same product and can't be upgraded from the former to the latter.

Resolution
In order to monitor Azure SQL Databases, use the new Cartridge for Azure SQL DB 5.9.5.x or higher.
This new cartridge requires additional licensing.
