One of the possible reasons might be related to Domain Controller Security Policy restrictions.
To resolve the issue:
1) Find out the Domain Controller specified as an Associated Domain Controller for the server where the agent is being installed. To do that:
a) Expand source or target organization in QMM Management Tree down to the Exchange server in question (Alternate Host server).
b) Right click the server in QMM Management Tree and select Properties form the shortcut menu.
c) Click on Associated Domain Controller page.
2) Log on to the Domain Controller.
3) Open Domain Controller Security Policy > User Rights Assignment > "Replace a process level token" policy
4) Make sure both LOCAL SERVICE and NETWORK SERVICE users are added to the policy above.
5) Start cmd and type in: Gpupdate /target:computer /force
6) Restart the "Windows Management Instrumentation" service on the QMM Project Manager machine.
