The attachment icon was not rendered properly in the Note field during migration. The icon was short and out of proportion.
This issue has been fixed in Migrator for Notes to SharePoint version 6.8.0.2372.
If you are on a version lower than this build and experiencing this issue, upgrade and run your migration again.
