FMS fails to start and connect to FMS database "account is locked" ORA-28000 (4249539)

Foglight web console is not accessible when Foglight Management Server (FMS) database account is locked.

ManagementServer log: 
FATAL   [main] com.quest.nitro.service.upgrade.db.DatabaseUpgradeService - Fatal error connecting to database. Shutting down Foglight server... Check and make sure your database configuration is correct and the database server is up running.

datasource-auto-thread-dump generate error: Could not create connection; - nested throwable: (java.sql.SQLException: ORA-28000: the account is locked); - nested throwable: (org.jboss.resource.JBossResourceException: Could not create connection; - nested throwable: (java.sql.SQLException: ORA-28000: the account is locked))

1. Unlock the FMS database account (to be performed by the database administrator).  Substitute actual username in SQL command below.

SQL> ALTER USER username ACCOUNT UNLOCK;

1. Restart the FMS.
