Is there a setting to stop the header from being added by the formatter?
Resolution

To get rid of the header that is inserted after formatting a script:
a) Select View | Formatter Options from the Toad menu bar.
b) On the left side, highlight General Layout | Header
c) On the right side, select Disable for the tagline.
