Unable to apply Oracle EBS Patch since the db kept on blank out after selecting it

When I try to apply an Oracle EBS Patch in Stat 5.7.4, I cannot get the fields filled in. I select my target environment from the drop-down and then select "Apply Patch". As soon as I select "Apply Patch" then my Environment field gets blanked out. When I go back to Environment and select my target, now my Action is blanked. I reselect "Apply Patch" and my Environment is blanked out again. If I select Patch Analysis as my Action, then it works. 

This issue only occur with csr that contain workflow

WORKAROUND:

1. Open up the workflow | double click on the csr status for status rules. Ideally, if none of the options is highlighted in the environment available for patch application, Stat should allows user to apply patch to all environment. However it's not behaving correctly in this case.

2. Highlight the db where user should be allows to apply patch. If all environment, then select all.

3. Save the workflow

4. Create a new csr
