A list item that has versions 4.0 and 5.0 is migrated to SharePoint Online (SPO). However, on the target environment, the list item has versions 1.0 and 2.0.
Cause
This is a known issue when using the Azure Migration Pipeline.
Resolution
As a workaround, use the Client Side Object Model (CSOM) adapter with the migration by disabling the following options:
